WELCOME:
I hope this information will answer some of the questions you have about my professional services and office policies. I would also like to acquaint myself with you and your desires for the work we are embarking on together. I am including some of the rights and responsibilities of engaging in treatment with me. I encourage you to read this document carefully. Together, we can discuss any of the information detailed here and any questions you may have.
ART THERAPY SERVICES:
I provide art therapy to individuals, children and families that are looking to make changes in their lives. We will collaborate together on the goals for your treatment. I will support you and assist you in reaching the set goals, but ultimately the responsibility for the success of the experience depends on your investment in the process. During each session, we will use the creative process of art making to improve and enhance your physical, mental and emotional well-being.
I offer one free 15 minute introductory session. During this time, we will both decide if I am the best person to provide the services you desire in order to meet your treatment goals. If you feel it is a good fit and would like to continue services, the following sessions will exist as 45 minute meetings. During our second session, I typically conduct an initial assessment that will assist in getting to know you better as a client and allow me to gain further insight into what treatment might be best suited for you.
PROFESSIONAL FEES:
Payment must be received at least 24 hours prior to the time of each session, unless alternate arrangements have been made with me, prior to our visit. No refunds will be issued and all payments must be paid on a timely basis in order to continue working with Erika Biesemeier, LCAT, ATR-BC. Payment per session or art therapy package is by Zelle, PayPal or Venmo. A statement for payment will be provided at the end of each month if requested.
APPOINTMENT POLICY:
Erika Biesemeier, LCAT, ATR-BC and you /the client agreed to keep all arranged appointments. Clients are expected to be on time for each appointment and cannot be allotted additional session time in the event of a late arrival. In the event that an appointment must be cancelled, both agree to give at least 24 hours notice and the appointment can be rescheduled. If you / the client does not show to an agreed-upon appointment time or insufficient notice is provided, the client will be responsible for payment for the missed session.
